Intense Headache During Nights. No Help With Migraine Medicines Taken For 10 Years. Remedy?

Hello SIr, I am Arnab Datta age 36 years. Recently, I am suffering from acute Headache (Back left side of my head). It normally starts middle of the night with intense pain in or around the eye on one side of my head (Left side). I think, it is Cluster Headache . However, earlier my local physician suggested that it may be Migraine (10 Year back) and prescribed Amaxyde H , which I took daily (2 Tab) for last 10 Years. But, there is no use. This intense pain started very recently lasting more than weeks now. Earlier, the pain was not so intense and occured rarely. Please do suggest remedial. Regards. Arnab Datta.

Neurologist 's  Response
Hello,
Thanks for the question
i am a neurologist
Let me tell you that a given medical problem can be due to various causes and it applies to headache too. Headache can also be due to various causes like brain tumor, infection, migraine, cluster headache, high blood pressure also. The main objective of the treating doctor is to find out the cause and deal accordingly. While evaluating a patient with headache we need to know the presence or absecnce of certain symptoms which will guide us narrow down the possibility. Foe example we need to know where is the headache, is it episodic ? does it come in clusters and then one has headadche free periods of months? vomiting or not any lacrimation or not and so on.
I advise to you will be to undergo a neurological assessment who will tell us the presence or absence of any neurological deficits and if a brain scan is reuired or not. You have to understand hat cluster headache and migraine are the dianosis of exclusion. I have a feeling that you may have cluster heacahe and you will benefit with the following medicines if the diagnosis is varified by a local neurologist
1. during acute attack- a short course of steroid will help you
2. During the chronic phase verapamil will benefit
hope this will help you
