Hi,

As per symptoms and signs of your child, it looks he is having otitis media with the perforated tympanic membrane.

You need to give him antibiotics as was prescribed but not for more than a week, side effects needs to be kept in mind as you said there are many.

Paracetamol oral suspension will be helpful in providing relief for severe fever.

Make sure he takes mothers milk as much as possible also make him hydrated with other fluids which he likes much.

In case of any sleepiness or dizziness you need to consult a ENT specialist for further evaluation.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
