Have Anal Fissure. Pressure While Passing Stool. Did Ano-rectal Surgery. Reason?

Hello doctor I'm Arijit from Kolkata. I've been suffering from anal fissure since last 3years..i did an ano-rectal surgery on 11november,2012. The doctor who did my operation told me to use an anal diluter for three months. But i'm feeling not okay as i've to give too much pressure for passing stool. I am afraid of any more surgeries. What should i do now?? Please help

Hi thanks for your question.
There are two types of procedure done for anal fissure.One is old type called as EFD in which sphincter is torn and in second type is called as sphincterotomy and this type sphincter is cut surgically.EFD heals by fibrosis and leads to anal stenosis which needs regular dilatation.Some amount of stenosis also occur in smphincterotomy.I would suggest you to reconsult your operating surgeon who may be able to examine you to look for amount of stenosis your having and to determine amount of diltation still you need.

Are you suffering from constipation?
There appears to be high possibility of constipation induced recurrent injury at anal area predisposing to anal fissure.
You should consult with internal medicine specialist/ general surgeon and should go for thorough check up.
You should also go for complete blood count, blood sugar, serum electrolytes, and thyroid profile.
You should take plenty of water, high fibre diet and laxatives containing lactitol and isapgula.
You should perform increased physical activity to maintain gastrointestinal mobility.
You should also take proton pump inhibitors along with prokinetics.
You should also apply diltiazem ointment at anal area.
You should also take hot sitz bath three times in a day.
