Question: hi I am a 53 year old male I have had low urine pressure for approx. 10 years , which whenever I have mentioned it to a Dr. they didn't seem to make a big deal about it saying my prostate was inlarged. in the last year I lost feeling in urethra cant feel pee coming out as much ,and lately I have had new symptoms. I had a few painful ejaculations. And all the sudden when I have sex with wife I don't feel the orgasm as much sometimes not at all, also my psa test was normal 0.1 if I recall that test was done 6 months ago. worried , also have cloudy pee sometimes, I have diabetes that I have recently gotten control of using long acting insulin, wife recently found out she is a hpv suspect we have sex 4 times a week for 29 years.

The fact that you have it difficult to control urine is an important reason for a good check. Prostate problems, diabetes, urinary infection or other neurological reasons need to be considered and ruled out as the cause.

You will need a urinalysis and urine culture to make sure if there is urinary infection, especially because your urine is cloudy sometimes (this can be due to crystals as well but a urinalysis would help distinguish). You may continue with cranberry juice and drinking plenty, it will flush your urinary system and prevent infection.

Your doctor will need to ask for HbA1C test (if not done in the last 3 months) to confirm diabetes is under control. Diabetes may cause neuropathy which may affect urination and ejaculation.
A rectal exam and possibly an ultrasound to evaluate the urinary system and the prostate might be needed.

Finally a good neurological examination might be advisable especially if you have had back problems as these may cause issues with the nerves fr the spine and problems with urine or bowel control.

So, it is good you are having a visit soon, the above things are some examinations that you might expect to happen. Also, very likely your symptoms are due to the prostate and there is good effective treatment that will help with urinary symptoms (treatment for benign prostate hypertrophy BPH) that your doctor may prescribe in case urinalysis comes out normal and glucose is well controlled.

I don't think your symptoms are related to HPV but your wife will need regular Pap smears to make sure her cervix is OK.

I do have a back problem, got hit in back 35 years ago with dirtbike but recently have been getting what I believe to be sciatica a pain in my butt cheek that cripples my right leg cant even walk a foot, I also have a appointment with a neurologist coming soon, one of my main concerns is the meds they give for prostate ,ive been told effect you sexually. are there prostate cures that wont put an end to my ability to have sex. thank you for your response.

Doxazocin for example has it less likely compared to 5-ARI medications and is one of the first to be started, so I would recommend you to talk with your doctor on the medications and get one that the risk on sexual activity is less.
