Question: hello doctor, my mother who is 57 years and diabetic has dizziness as soon as she wakes up in the morning. This dizziness doesn't last long as it reduces by noon. This all started 2 years back when she fell and injured her head. CT scan was done which showed normal . But it turned into vertigo from then. She was treated with eplys maneuver and some medications for which she became fine. Now the dizziness has started again since a week and she taking vertin 16mg tablet which is not helping her. Please suggest which helps her reduce dizziness especially in the mornings. Eagerly awaiting your response..

Hii. Welcome to the healthcare services. She is diabetic and has history of head injury. It seems that she is having positional vertigo or BPPV for that epleys manouvre was done. Now she is having again vertigo. So now check blood sugar, HbA1c, creatinine and a complete blood count. Check her blood pressure. Do a ecg. If possible consult an ent surgeon and repeat the epleys manouvre. If you can't find an ent surgeon then increase the vertin dose to 24 mg twice daily for 2 week. Don't sleep on the side where giddiness appears. Keep injection stemetil and emeset on standby. If giddiness increases then you may have to give them. Tell her to take complete bed rest.
