Question: have a long history of heavy bleeding and pain with clotting with my period since my second child in 2000. I was treated for Endometriosis in 2001 or 2001. They did laparoscopy back then. After I followed up with my GP, he told me they got most of the endometriosis but could get what was on the "pouch od David". I was much better aft er the surgery. In 2003 I had my third child. between 2003 and 2015 I have had two miscarriages and two abortions. I am not sure when the heavy bleeding and pain came back but I have dealt with it for at least 7 years. Last March (2018), I went on Depo to stop the bleeding. This worked up until about three weeks ago. The bleeding and pain is still less but has been on going for about three weeks now and is heavy but spotting. I have lots of cramping. I have back pain. I have clotting. I have lots of clear discharge, I am also have loss bowel excrete from my vagina, it has happened about three times now in the last month where I thought I was just having a gush of period and when I go to the bathroom it fences

I forgot to mention I have also been diagnosed with PXE. I have tossed vision in my right I due to this disease. I see a Ophthalmologist for this condition and was seeing a cardiologist as well.

Brief Answer:
Can be fistula and endometriosis.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The continuous bleeding for last three months is worrisome. The passage of faeces from the vagina is also worrisome. This can be related to a fistula. A hormonal imbalance and relapse of endometriosis also needs to be ruled out after a few investigations. You need to consult your gynecologist for a detailed examination and for prescription hormonal medicines to control the bleeding.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Thank you for your reply. I also looked online and this is what I saw as well.

I have only been bleeding for three weeks not three months, is this still concerning?

Any chance going on Depo at age 47 could have something to do with these symptoms?

Can you tell me how a Fistula might develop with out any trauma?

Could the Endometriosis on the pouch of XXXXXXX have anything to do with these symptoms? Can Endometriosis come back? How long does it take for it to come back?

I had an ultrasound on Friday, should have results this week. Also doing blood work and urine analysis Monday.

Can you tell me what things I should tell my GP that would help them send me to the right specialist?

Brief Answer:
Details below.

Detailed Answer:
Hello.
Thanks for writing again.
Bleeding for three weeks is also concerning and needs a proper treatment. Going on Depo can cause a mild persistent spotting but not a heavy bleeding with clots.
Fistula is unlikely to develop without a trauma but passage of feces is suggestive of fistula only and needs a close examination to find the cause.
Endometriosis can cause a persistent pelvic pain. It can relapse any time.
You need to tell about the bleeding and passage of feces to your GP to get right referrals.
Hope my answer is helpful.

Thank you. Going to see GP as soon as I can this week. I will explain other symptoms to them and hopefully get a referral to a GYN. It's hard to get some doctors in my area to take your symtoms seriously. In their difference, I am not very good at diagnosing my symptoms cause, one, I am not a doctor so unless I google it I don't know it's not normal and, two, I just always chalk up my symptoms to being normal or nothing to worry about, most of the time. I will have other questions as I go through the testing and procedures if they discover something. Appreciate the advice and the back up.
