Question: Switching from OxyContin to Oxycodone ER. Currently taking 40mg of Contin every 4 hrs. What should be the dosage intervals of Codone ER @ 40mg each?

Oxycodone and Oxycontin both are same but one is extended release tablets. It can be prescribed 5-30mg every 4-6 hours oral preparation. But respiratory depression should be monitored. Usually extended release tablets will come 10mg every 12 hours. For hydrocodone it is 5mg every 4hours. I hope I have answered your query about opioIds. Paragraph 2 you state...5-30mg every 4-6 hours of Oxycodone ER. Are you stating a total of 30mg? I don’t understand the “5”....my tablets are 20mg Oxycodone ER.
My current dose of 40mg OxyContin is every 4 hours.
I want comparable pain coverage using Oxycodone Extended Relief...could you please state you recommendation using 20 mg Oxycodone Extended Relief Tablets, thank for your understanding

Hello sir, first of all, I would apologize for not being clear. As far I know we are using 5mg, 10mg, 20mg up to 30mg oxycodone dosing every 4-6 hrs given here depending on patient's pain and other factors like if he is in-patient or resistant then higher dosing is possible but we decide all that depending on the patient. The tablets I talked about just now are not extended-release tablets.

Extended-release means acts far more time than this oxycodone alone. They will not disintegrate in the body soon and needs more time for release. Here we are using 10mg extended release tablets every 12 hours only. I don't know who gave you 20mg. It should be used preferably 24hrs apart only. If you want to use 12 hours apart can also be recommended but needs monitoring for side effects and also respiratory depression. so if you take 20mg tablet 12hours apart kindly get back to the nearest hospital if you feel anything problematic. 40mg, 60mg and 80mg tablets are reserved only for in-patients and resistant patients only.


So you can take a 20mg tablet every 24hours safely 0r 12 hours if you monitor for respiration and other side effects. I hope I have explained you properly.

I am considered high resistance. One might say I am a high functioning addict. My exoskeleton is twisted with chronic lower spine pain 7-9.5 on a 10 point pain scale.
I was taking 260 mg daily of OxyContin and requested a comparable time released ER product to insure a better balanced dosing as my pain has been uneven in the past year.
My doctor prescribed 20mg ER OxyCodone, but the Chemist simply put the former dosesage on the new product. I knew my doctor was not available due to travelling to seminar in Hong Kong and Singapore, I tried email. In a fit of desperation I decided to try an on line service.
I have since been put on something called ER phasing. I have been able to reduce my daily dosage by 40 mg with a 60 to 80 mg target. I am optimistic and I can understand the unorthodoxy of my question. Thank you.
