What Is The Reason For Cloudy And Ammonia Odored Urine?

Question: Symptoms: 3-4 weeks post op of Anterior/posterior approach spinal fusion at L4-L5 my abdomen seemed to fill with fluid. Pain clinic provider said see surgeon. Surgeon said see Primary. Looking for new primary, need an Internist. Meanwhile, surgery was 1/8/18 and its now May. I had been in a temporary housing situation and have now landed. Last week finally saw a CNP at local clinic. New symptoms now include very cloudy, very ammonia smelling urine for about a month. Just before going to clinic also began foul smelling along with ammonia smell. Severe back pain that I've been attributing to post surgical pain but maybe it isn't? Finally had Ultrasound sound today. Had to beg, seriously beg, to get CNP to order it. She wasn't in so someone else called and left results on vmail. No mention whatsoever of fluid. Just some fat deposits on liver. I'm a large woman who recently lost 30 pounds. Fatty deposits, unless I'm mistaken, are a given. My next step, other than firing the CNP, did I forget to mention that I had to beg for basic blood work also? She asked like what? To which I responded, oh, at the very least a BUN and Creatinine. After she consulted with what I have to assume is her supervising MD, she came back in and said, Oh, I think well run a full Metabolic after all. As you can see, I'm having a hard time getting good care here, and I obviously need something! Hoping you can help! Or, if not, point me in the right direction.

Hello,

I can understand your concern. As you have cloudy and ammonia smelled urine, causes of that are urinary tract infection, dehydration, bladder stone, renal stone. Ammonia smell mostly due to concentrated urine. Let me know whether do you have fever, burning micturition, blood in urine, abdominal pain, frequency of micturition? Kindly follow up with detailed history.

I'd suggest you to do urine analysis, urine culture and sensitivity and complete blood count, creatinine, liver function test. There is nothing any significant finding in your ultrasound abdomen report. Fat deposition in liver is not significant finding until you have normal liver function test.

You may need tab. nitrofuratoin 100 mg or tab. cefpodoxime 200 mg for UTI after consultation. Along with that you can take syp. Disodium hydrogen citrate 10 ml. As these are the prescription medications, kindly consult with your doctor. Kindly do urine lab. test before starting antibiotics. Keep yourself well hydrated.

Regarding severe backache, Kindly consult with your spinal surgeon. For pain you can take tab. tylenol 500 mg. Apply diclofenac gel or ointment locally. Do regular physiotherapy after consultation with physiotherapist and surgeon. I think this will clarify your concern. take care.
