What Is The Endometrial Cut Off For A Post Menopausal Woman?

default
Posted on Sat, 24 May 2014
Question: I am a 35 year old female, post-menopausal (since 12/2012), currently on continuous HRT (until 5 days ago when my doctor asked me to discontinue it) since XXXXXXX of 2013. I had a uterine ablation done in May of 2010 also. I have been having some off and on bleeding since January of this year, so my doctor wanted to get an ultrasound to evaluate things (why I had to temporarily discontinue HRT). An enlarged uterus and cervical bleeding was noted on exam. I had the ultrasound done on Monday. The tech, of course, cannot tell you anything; but she did say that everything looked pretty normal and that the doctor would call me. I haven't heard anything yet. My question is, what exactly is normal? I wonder if she meant normal for my age or normal for someone that has had a uterine ablation and is post-menopausal on continuous HRT. What would the expected measurement for the endometrial stripe be in a case like mine?

Hello
Thanks for writing to us with your health concern.
Before I answer you, I would need to know a few things -
1. Why are you post menopausal, and have been so since the age of 32 ?
2. What HRT were you taking ?
Now, for a post menopausal woman, the endometrial cut off is 4 mm.
Anything, above that measurement is considered abnormal.
That cut off is valid for anybody who has had an ablation also, but not for someone on HRT, as then, the stripe can resemble that of a normal menstruating woman, and should be less than 12 mm.
All the best

If you can, please upload the scan report.

Premature ovarian failure is the reason for premature menopause. I take Climara Pro. It is an estrogen + progesterone/progestin, continuous regimen (one patch per week).

Detailed Answer:
After how many days of stopping the patch ( before inserting a new patch), did you have the ultrasound scan done ?
You would have had some kind of withdrawal bleed after discontinuing the HRT ?
FOr somebody who has stopped HRT 5 days or more ago, the cut off values remain the same for normalcy, as far as the endometrial stripe is concerned.

I stopped the patch on Friday and had the ultrasound the following Monday (3 days). I have not experienced any withdrawal bleeding. I have been off the patch since Friday (5 days today).
So, because I have had the ablation, the stripe should be 4mm or less, regardless of HRT? Or, because I was on HRT (despite ablation and menopause) it is fine if it is thicker?
I am just getting anxious. My hot flashes have started again and I really want the hormones back.

Detailed Answer:
Even if you were ablated , and then had HRT, or you were on HRT which was stopped 3 days back, in any which scenario, more than 4 - 6 mm would warrant further investigation.
