What Does This Urine Test Result Indicate?

Question: I was looking at my urine tests results that were done in February 2015. My local clinic has an online patient portal so that we can either request appointments or review our medical records. Microalb/Creat Ratio, Random UR tests show some results that I keep forgetting to ask my doctor for explanation.

Creatinine, Urine Value: 95.3
Microalbumin, Urine Value: 67.6
Microalb/Creat Ratio: 70.9

Based on the reference ranges for all three, they seem to be low. What does that mean for me?

I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

1. Creatinine levels itself are variable and it varies dramatically based on the water intake (hydration status of the patient at the time of the test) so they are not so much significant and not much meaningful. That's why your physician himself didn't bother to mention it with you. Also different labs have their own reference ranges. Usually for your age group a woman has a value of 15-20 mg/Kg/Day. So it also depends upon body weight and is taken on a per day basis too. SERUM CREATININE is rather more specific a test and shows the glomerular filtration rate showing blood supply status to kidneys and kidneys' filtering capacity etc etc.


2. Micro-albumin test in urine is done for hypertensive and diabetic patients. if there is increased levels of the albumin it means that these diseases have done damages to the kidneys and needs attention. It is done serially after 6months to 1 year in diabetics and the hypertensives.


3. Albumin/Creatinine Ratio (ACR) is also done to see the same bad effects of the diabetes and hypertension on kidneys of the chronically ill patients. Again very high levels show damage to the kidneys and there is then a need for change in treatment regimen.

You have no raised levels of them and it shows there is no damage to the kidneys due to other comorbidities.

Thank you, Dr. Hanif! Just so you know, I am currently taking Lisinopril and Hydrochlorothiazide for Hypertension as well as Atavorstatin for Cholesterol. I also suffer from Lymphedema in my legs and feet. Thankfully, the treatment for that has shown tremendous improvement. I am curious to know whether or not damage in the lymph system can also be a problem for the kidneys in the long-term - even with compression wraps and/or lymphatic drainage massage.

This oedema actually is due to hypertension.

Detailed Answer:
I welcome you once again with a follow up. I appreciate your efforts of getting healthy and caring for yourself. I will explain the things in a simplified way to help assist you through the case.

When blood (hydrostatic) pressure inside the vessels increase they leak outside and causing pooling of fluid in dependent parts of the body (legs feet etc).

So this all is actually due to the fact that hypertension affects blood vessels too. Same way, kidneys are affected by the hypertension. These all are interlinked actually. What we have to do is, we have to drain that extra pooled fluid out of the system and it is done with Water Pills and also we have to decrease this pooling of blood in the first place, so you are kept on other anti-hypertensives too so that your blood pressure remains in such limits where fluid isn't allowed to be leaked much outside blood vessels.

Pressure stockings will aid in the work of HCTZ and other anti-hypertensives in this management.

As long as your blood pressure remains under check; your pooled fluid will remain under check; your kidneys will remain less vulnerable to injury due to high pressure. This is the triangle in your case, all interlinked; with HYPERTENSION being at top corner of triangle.
