You have multiple but small fibroids and increased bleeding and pelvic pressure so you definitely need to interfere with the growth of these fibroids in order to avoid future problems.
Ideally these should be removed surgically in the form of hysterectomy but since you are perimenopausal and had ovarian cyst in past so the symptoms can be due to these problems also.
There is a new drug in market these days named ulipristal, I have had very good results in my patients when I tried it on the ones in perimenopausal transition so you can discuss it with your gynecologist and take it as per advise as it is a prescription drug only.
For heavy periods you can take tranexemic acid which gives just a symptomatic relief and is not the cure.

Most common problem is excessive bleeding and especially intermenstrual bleeding
Also there can be polyp formation or degeneration of the fibroid tissue leading to pain and urinary symptoms.
Also in case it grows enormously (though less chances as you are in perimenopausal age) then operability gets complicated.
