What Does This TSH Report Indicate?

Question: Hi, I will like to ask an ebndocrinologist about my thyroid function.

I went to the immergency room about two weeks ago, complaining about left side of my face being numb, and severe headache, and blur vision. They did a MRI on my head and confirmed that I did not have stroke. One of the doctor was mentioning that it might be thyroid problems. I had both of my thyroid removed in 2007 and currently on synthroid 88 mcg. My recent blood work reviewed my TSH is 7.86uiu/ml The normal range according to them is between 0.45 - 4.500 and my Thyroxine (T4) is 8.3. normal range is 4.5-12.0. so, My TSH is high, so that mean I should be on a higher dose than 88 mcg, because I am really not feeling good, beside being tired all the time, I have no energy and I feel cold and hot at the same time. My question is should I increase my doses of synthroid? or the result above is normal? your answer is most appreciated.

I am 66 year old, female, weight 97 lbs, and I am 5 ft. tall.

There is another questions that always puzzle me. Is there a different if I do blood work for Thyroid in the morning vesus in the afternoon?

According to your weight, 70-75 mcg a day of synthroid is good enough. However, some people may develop resistance to synthroid and may require higher doses.

I would say, recheck TSH again after a week or two before deciding about dose escalation.

Yes, TSH tend to rise towards late evening and i generally advice my patients to do the test in mornings.

I am a little confused here. My latest TSH is on the high side. My recent blood work reviewed my TSH is 7.86uiu/ml The normal range according to them is between 0.45 - 4.500. So it is high, but it also mean 88 mcg is too low for me. All I understand blood work for TSH is that, if TSH is high, that mean, I am low in Synthroid, it should increase the doses. However, you think i should reduce my Shynthroid to 75mcg. I am totally confused. should I do 2 blood works, one in the morning and one in the afternoon to see the different?

I know everybody's range of measuring TSH is different. Can you tell me what is normal range for me, TSH, Free T4, FreeT3 and Thyroxine (T4) considering I don;t any thyroid? I have both removed in 2007.

hello,
Sorry for the confusion. What I meant was that, according to your weight, the usual dose is 75 mcg. The dose calculation is 1.6 mcg per kilogram body weight. However, some people may require more than this dose like in your case.
Your TSH is 7.8 which is high and that means you may need to increase the dose of synthroid. However, I would strongly recommend to recheck only TSH in the morning and then let me know. No need to do again in evening.
For most labs,normal TSH range is between 0.4- 4.5 .
I would like to maintain TSH around 2.5 to 3 in my patients.
