Posted on Fri, 16 May 2014
Question: Knee pain with stairs, posturual etc. with history of pedestrian accident. Any suggestions with recent MRI shows the following:

Patellofemoral compartment: Moderate to severe narrowing of the medial articular cartilage with near-complete denudation of the central portion of the medial facet. Moderate narrowing of the apex of the patella with an area of delamination at the apex. Underlying reactive marrow edema at the patellar apex and medial facet on image 3-11. There is a deep fissure and possible delamination of the cartilage within the lateral trochlea with underlying marrow edema on axial series 3 images 16-18. Mild fissuring within the trochlear groove with underlying marrow edema also noted on series 3 -18 and 7-15.

Brief Answer:
Do quardricep strengthening exercises.

Detailed Answer:
Hi,

I have gone through your query.

From your history and MRI findings it is clear that your cartilage in knee and patella has been damaged. So, I would like to suggest you to do quadriceps strengthening exercises two times a day as uploaded on you tube and take 1 tab. aceclofenac +thiocolchicoside and three times a day.

Apply hot packs for 30 mins in morning and evening.

Try to avoid squatting, upstairs, jogging as much as you can. But you can do your morning walk.

If pain still persists after 2- 3 days, then, you should get ready for the minor procedure i.e Debridement and Lavage via diagnostic arthroscopy.

Now a days other important procedure also come into place i.e Autologous Chondrocyte Implantation but these area to be done by trained surgeons.

I hope you have got your answer.
