Question: type 2 diabetic o humalin U500 (fixed doses) and Humalog for adjustments. Diagnosed a month ago with pancreatitis--numbers have almost doubled. Recent CT scan shows "bilateral cortical thinning of kidneys". Lipase 140 (range 0-59), GGT 301 (range 0-60), uric acid 7.5 (range 2.5-7.1), serum chloride 94 (range 97-108), AST 40 (0-40), ALT 50 (range 0-32) [both AST and ALT steadily increasing since August], cholesterol 202 (range 100-199), triglycerides 561 (range 0-149), Alkaline Phosphatase 142 (range 39-117). Pancreatitis is causing extreme insulin resistance. CTscan showed some hepatic steatosis and some enlargement of the liver. Gall bladder and liver ducts normal. Not sure what to check next. Sometime in the past month I passed 2 small kidney stones.

Brief Answer:
High triglyceride levels need to control immedniately

Detailed Answer:
Dear friend, greetings form HCM. I understand your problem. Yes very high triglyceride , that is the reason for your pancreatitis. You need to take medications to reduce that first. All the parameters reveal pancreatitis. Yes diabetes will get worse , need to adjust the insulin levels accordingly. Diet must be lipid free .please let me know the medications you are taking for high triglycerides . I think you are alcoholic as revealed by your GGT levels , must stop smoking and drinking alcohol at this time. Meanwhile, not to worry too much about that renal stones . Think you need more than investigations more of monitoring and regular follow up. Thanks a lot .

I have never smoked and rarely drink (maybe 1 drink a month--and I quit even that several months ago). I am taking Livalo for cholesterol and Tribenzor for BP. I had to stop taking Crestor, etc. due to side effects. Diabetes has been out of control for some time--before pancreatitis (but then they never checked my lipase and GGT before). After diagnosis I cut out all fried foods and went on low fat diet. I am allergic to dairy and beef, so I drink almond milk and stick to poultry and fish. I'm seeing my cardiologist Tuesday, and GI doc on 12/21, and endo on 12/23. Sounds like I need to be even more aggressive about cutting any type of fat from my diet. Any other suggestions?

Brief Answer:
That s fine you be under regular follow up which is very critical

Detailed Answer:
Dear friend, Thats finel; follow up with your physician regularly and take the medications properly. Rule out galstones that could be the cause for GGT if not alcohol. Anyway please see that you get a scan and CT again take a second opinion.

THANKS A LOT. Please rate this answer before you close.
