Question: I have this red area on my left buttock. It doesn't hurt, is smooth, not scaly.

I have gone through your query and I have also seen the Image

This looks like a purpuric patch/ecchymosis in the Image.
I will keep a most likely possibility of Senile Purpura/ecchymosis. This is just like a superficial bruise.
These can happen due to capillary fragility and may arise even on the slightest of trauma or scratching or rubbing.
I would suggest an oral Vitamin C supplement once daily for 2 weeks. It should resolve within 10 days to 2 weeks.
