Let me clarify you about size of penis.

Size of penis varies according to individual. In flaccid state it ranges from 2-4 inches and in erect state 3.5-6 inches. Normally minimum 3-3.5 inches (8-9cm) penis in erectile stage is required to satisfy female partner. 4 inch penis size is normal in your case.

There is no approved medicine/procedure which can increase penis size in normal state. Only surgery can do the same but that is only indicated for medical reason not for cosmetic or sex purpose as there are complications and risks associated with the same.

You mentioned, “cant cum when i have sex are masterbate ". From this I understood that there is difficulty in ejaculation while you do the sex or when you masturbate.

No orgasm and no ejaculation mean anorgasmia.

Detail evaluation for anorgasmia is required.

- Did you have history of nightfall?

- Are you using any medicine for anything currently?(apart from thyroid)

- Do you have any problem in urination?

- Do you have nay backache or spine problem?

You need to consult urosurgeon for evaluation.

Testosterone level should be checked.

Local/per rectal ultrasound also needed to check anatomical structures.

If everything come normal local neurophysiological tests (bulbocavernosus evoked response and dorsal nerve somatosensory evoked potentials) should be done.

In my opinion consultation of urologist needed. If urologist finds everything normal, opinion of psychiatrist with detail history and examination is to be taken to check for psychogenic cause.
