What Does My X-ray Report Indicate?

default
Posted on Thu, 11 Dec 2014
Question: I have little swelling in left hand. The scanned copy of x-ray, prescriptions and medicines taken are attached. My Doctor is suggesting taking an injection directly on the vein which I am skeptical about. Please suggest appropriate remedy for this problem.

Detailed Answer:
Hello
It’s good to see you
I have seen the attached papers and would like to know –
Do you have pain at base of thumb near wrist with difficulty in making a tight fist with thumb inside the palm?
Severe pain in showing thumbs up position?
If answer is’ yes’ then most likely diagnosis is Deqervain tendinitis. In this there is inflammation of tendon (Tenosynovitis) attached to thumb and pain occurs when thumb /tendon is under stress.
My suggestion for you would be-
1. Avoid lifting heavy weight items like pressure cooker, Iron .you may do Ice fomentation.
2. Drugwise tablet.Tendomac morning for an evening for 6 weeks, along with pain killer like voveran SR 75 mg on as and when required basis.
3. Phsiotherapy in form of TENS therapy helps.
4. In cases not responding to above treatment injection Depomedrol is a safe alternative, if given in the right way to avoid tendon rupture.
You will get over it but it does take time (6-8 weeks or may be more) with medicine and physiotherapy.
Hope this suffices and I shall be glad to answer further query.
Take care

Dear Dr Chugh,
I am able to make a fist with the thumb inside and also able to do thumbs up with no pain. Actually I have pain when I try to lift a little heavy weight or when my hand gets a little jerk. The pain is on the wrist area itself with little swelling on the nerve. Attached is a pic in which the pain area is marked.

Thanks

Brief Answer:
As follows

Detailed Answer:
Hello
I have gone through your reply carefully. You may be having pain on outward movement of wrist with thumb inside the palm(Finkelstein test).
Based on the area marked and with difficult and painful thumb motion, particularly when pinching or grasping objects. It is suggestive of de Quervain's tendinitis.
Treatment of de Quervain's usually involves wearing a splint 24 hours a day for four to six weeks to immobilize the affected area and refraining from any activities that aggravate the condition. Ice may be applied to reduce inflammation. As I told you earlier it is safe to have injection of steroid to decrease pain and swelling
Hope this suffices.
Take care.
