default
Posted on Thu, 26 Feb 2015
Question: SFA test was done 4 times during last 3 mouths. SFA test result is abnormal. There is higher range of pus cells. Also Total Sperm Concentration and Active Motility are poor. Semen culture Test was done recently. Semen culture Test is normal (Microscopy is Nil). I have 5 year old baby and I am having problem to get new baby in last 3 year period. All Test reports are attached.
My questions are
1.     What is reason for higher range of pus cells and poor Total Sperm Concentration and Active Motility?
2.     What are additional investigations and Tests I have to be done?
3.     How can I get cure for the problem of getting new baby? What are treatments?
4.     Is this STDs? And if so can it transmitted to my wife? What do I have to do for this?

Brief Answer:
Needs further evaluation.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that your recent semen examination has revealed plenty of pus cells and low count of grade one motile sperms. You are trying to have your second baby since last three years.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have any urinary symptoms like burning ,frequency etc ?
2) Did you have sexual intercourse with other female unknown to you?
3) Do you have pain while ejaculating?

Though your semen examination revealed plenty of pus cells which strongly suggests infection of either Seminal vesicle or Prostate it is rather curios to find no growth of organisms on culture.

Please consult qualified Urologist and get following basic tests done to find out the organisms and cause of this infection
1) Repeat Semen culture in other laboratory after abstinence for 3-4 days.
2) Trans Rectal Ultrasound Scanning (TRUS).This will help to know the status of Seminal Vesicles and Prostate.
3) MRI of pelvis.

In the mean while take broad spectrum antibiotics like Doxicycline or combination of Trimethoprim and Sulfamethoxazole twice daily for 4 weeks and repeat your semen analysis and culture after one month.

Low count of grade one motile sperm is due to infection and it will increase once your infection is controlled.

Hope I have answered your query ,please feel free to ask if you have more questions I shall be happy to help you
Thanks and Regards.

Brief Answer:
Get Basic screening blood tests for STD done.

Detailed Answer:
Hello
Thanks for feedback.
In view of sexual intercourse with other unknown woman and you are not being aware about her STD status it is better you get your basic blood test for STD done.

Continue taking medications which I have suggested in my reply earlier and repeat your semen analysis after one month with due precaution of observing abstinence for 4 days before collecting semen sample for test. .

Thanks and Regards.
