Question: Dear Dr Taka,
You have answered one of my queries a few weeks ago. The link is below to refresh your memory.

Since this post my family member has had a MRI. The neurologist called and said that he will need hindbrain surgery very soon. I am yet to find out the details as the message was recieved with little information and the offices are now closed for the holidays. I was just contacting you to see if you had an idea what this could be from the information I have provided and if you where available to ask any questions when I find out anything more. You have been very helpful, more so then every doctor we have ever seen combine over the past 40 years .

Hello and thank you for the update.

To be honest I am a little puzzled. I do not really see what surgery could be useful for apart from a shunt revision if the pressure is mounting again.

Otherwise it could be an independent issue found by chance, something undetectable by the CT scans but evidenced by the MRI. Looking at the hindbrain on the past CTs, the venous sinus system (the venous sinuses are the big veins, vessels through which the blood leaves the brain to return to the heart) on the center right are enlarged. So perhaps the MRI might have revealed changes in other vessels in the area, a vascular malformation. But there are often variations among individuals and it was the same from the 2009 scan to the 2015 one, so not evolving. But it's a remote hypothesis I'm making, mentioning it only because it's the only unusual thing I notice in the hindbrain. Also (if that was the issue found by MRI) it isn't responsible for the other previously discussed issues in the anterior part of the brain (corpus callosum agenesis, hydrocephalus etc).

I am sorry if my answer is somewhat vague, but I hope you understand given the lack of the MRI report. Feel free to use your free follow up question once you have the report.
