Brief Answer:
She needs to consult gynecologist.

Detailed Answer:
Hi
Welcome to healthcare magic. I am Dr Ramadevi Wani. I will be answering your concerns today.

Since when is she having this on and off spotting? Is it related to coitus? Has she been regular with her Pap tests?

There are several reasons for this spotting
1. Infection
2.Cervical erosion
3. cervical or endometrial polyp
4. Rarely due to precancerous conditions of cervix which can be treated completely.
5. Hormonal imbalance ( sudden change in weight, stress etc can cause hormonal imbalance).
6. Blood thinning medicines like aspirin, warfarin etc if she is taking them for lupus

I suggest that she consult Gynecologist for internal examination, ultrasound and may be hormone tests too. Accordingly treatment can be given.

I hope this is helpful.
If she has further concerns, do write to me.

Best wishes,
Dr Rama
