What Does Bleeding During Pregnancy Indicate?

Question: Hi Doctor,

I came to know 07Jan2015 that my wife is pregnant but bleeding(blood spot) occurs from 10Jan2015.

On 3rdMar2014 bleeding occurs more then we have got UDG on 4th March and then we came to know about baby is normal but but with irregular cystic structure about
20X12mm is seen in left cornu of uterus, seprate from left ovary - possibly due to unviable gestational sac.

We got one more USG report at 30March2015 and report is normal(but with irregular cystic structure about 20X12mm is seen in left cornu of uterus, seprate from left
ovary - possibly due to unviable gestational sac):
LMP: 31 Dec
CRL: 6.6
Gestational Age: 12 wk 6d
Placenta: is posterior in location.It is grade -0 in appearance.
our consult doctor told use to not need to worry about irregular cystic. She was saying it will desolve automatically or come out thru vaginal.

On 02April2015, bleeding occurs with dark red and some red(with 3 pieces) so we and our doctor thought that it is irregular cystic so we have got one more USG today
04April2015 and report was almost same (now small irregular cystic structure is still exists) as above I wrote but it is showing about Placenta poisterior low lying.

Hi XXXXXXX govil, I have gone through your question and understand your concerns. Bleeding and spotting in first trimester can be due to causes like:
1. Threatened abortion
2. Ectopic pregnancy
3. Bleeding around the foetus( subchorionic haemorrhage or irregular cystic lesions in left uterine cornu, as in your wife's case)
I would like to know whether your wife has a bicornuate uterus, diagnosed earlier or in this pregnancy. That irregular cystic structure in the left uterine cornu can be a non viable gestational sac, and it will resolve with time, as your doctor said. The cause of bleeding in your wife can be that sac or the collection of blood around it, which is slowly coming out through vagina.
Low lying placenta at this gestation( 13 weeks3 days) can be a rare cause of bleeding. But you need not worry as the placenta migrates upwards in the uterine cavity as the pregnancy advances.
In the meantime, ask your wife to take rest, avoid heavy and strenuous work, and avoid intercourse.
Your wife should be given progesterone in the form of capsules or injections for supporting the pregnancy.
I would suggest you to not to worry as the fetus is growing appropriately to the gestation age.

Hi Doctor,

Thanks For quick response.

irregular cystic structure is identified with this pregnancy on 4th March2015. My doctor is already giving progesterone (Gestofit 300 mg SR) and given one injection also HUCOG-5000 HP per week upto 13 weeks.

I want to tell one thing here also is that Cardiolipin Antibody IGM came out 25.10 and it should be less than 12.50 as per our doctor told us. So our Doctor suggested us to inject LOMOH-40(Enoxaparin injection IP 40 mg) alternate day.

Hi XXXXXXX govil,thanks for responding. Progesterone capsules and hCG injection (HUCOG-5000) are given in bleeding or spotting in first trimester of pregnancy.
Cardiolipin antibody IgM more than 40 MPL units is taken to be positive in pregnancy, and in that case enoxaparin can be started. Moreover, cardiolipin antibody is done in interpregnancy period and not in pregnancy, as there are altered levels in pregnancy due to altered immune response.
In my opinion, there is no need to start enoxaparin in your wife at these levels of cardiolipin IgM antibody levels.Moreover, enoxaparin makes the blood less coagulative, thus increasing the bleeding tendency and your wife is already having bleeding episodes.
