What Does A Drop In ALT Levels While On TB Treatment Indicate?

Question: Hi, This is Shagird from UK regarding TB treatment. I have been to the Dr. My wife's CT is completely normal. He said because her skin test was quite positive he recommended to do latent TB treatment. He also advised that before doing fertility treatment it is better to complete latent TB treatment to avoid any chances of becoming it active during pregnancy and causing more trouble. So we said ok. He prescribed Rifampicin and Isoniazide for 12 weeks. I think we can wait for fertility treatment 12 weeks and complete this course first. What do you think?
- regarding my treatment: my ALT has dropped to 50. He said it appears Pyrazinamide was causing liver problem. So they will keep pyrazinamide out from my treatment and continue with moxifloxacin together with Rifampicin, INH and Ethambutol for two months and then change to only Rifampicin and INH. They will not reintroduce Pyrazinamide. As treatment seem to have worked I could not argue about moxifloxacin. He said Moxi is better than Levo and kills bacteria quicker. I hope they are going ok. What do you think?

About your wife, no harm in taking rifampicin and isoniazid. Also wait to complete these drugs and then start infertility treatment.
About you, it is good that your ALT level is normal and not increasing. And yes, you should continue moxifloxacin instead of pyrazinamide for 2 months.
He is right that moxifloxacin is better than levofloxacin but moxifloxacin is reserved drug for XDR tuberculosis. So in XXXXXXX we are not giving to all tuberculosis patients.
