Brief Answer:
You are having diabetes Start on medications

Detailed Answer:
Hi,

Welcome to HCM. I have gone through your question and understand your concerns.

You are having diabetes and the sugars are too high to be just on life style modifications alone. You have to start on medications immediately. Glycomet (Metformin) is the starting medication for diabetes and is relatively safe.

You can start on this medicine and retest FBS, PPBS after 2 weeks. I recommend you to do a blood test- HbA1C (3 months' average blood sugar) to be done along with that.

Hope this will clear your doubts.

Brief Answer:
Discuss with your psychiatrist and diabetologist

Detailed Answer:
Hi,

Welcome back. I understand your concerns.

The decision of weight lowering medications can be made only after discussing with your psychiatrist and diabetologist/ endocrinologist.

I shall guide you through the options available.

Your obesity can be due to many factors like : genetic, lifestyle changes, diabetes induced weight gain, depression, medicines for depression etc. Some are modifiable and some are not.

The options of weight reduction include

1. Lifestyle changes - Diet + Exercise + Meditation
2. Medications
3. Surgery - bariatric surgery

Medicines useful in your case are:

1. Metformin - cause only mild weight loss (1-5 kg)
2. Orlistat - useful in patients who eat a lot of fatty food - gastric upset is common side effect
3. Victoza - an injection which can control sugars and cause weight loss of around 3-8 kg. Expensive, gastritis common.

You can discuss these options with your treating doctor and decide
