Question: My BP is 137/57 pulse is 87 is this ok
I currently take 7, Percocet 10/325, daily. Hi am has ng s hop replacement in 3 weeks. The Percocet is not as effective as it once was as I have built a tolerance. I have been on Percocet sincee March when I broke my femur and both bones in my arm from a fall. I can get down to one Percocet a day. How long will it take for my tolerance to decrease so I will have better pain control post operatively.

Hello!

Welcome back on HCM!

I would like to confirm that your actual blood pressure values and hear rate are within the normal ranges.

So, you have nothing to worry about it!

Your actual therapy seems to be really effective in controlling your blood pressure values.

Regarding Percocet, the dose that you are taking is really high. I would recommend to gradually reduce the dose with one tablet every 3 days. When arriving to the dose of 3 tablets per day, you can add 1 tablet of simple acetaminophen 500mg (Tylenol) in order to help control the pain, if necessary.

I think that with this gradual reduce in the dose , you will not have withdrawal symptoms.

When arriving at the dose of 1 tablet Percocet per day, you can add also two tablets of acetaminophen 500mg, to help you with the pain.

Only oxycodone can cause addiction and tolerance, while acetaminophen has not these adverse effects.

You can raise the dose of Percocet if necessary after surgery (reducing the dose of acetaminophen).

Xanax 0.25mg can be helpful during this situation, to help with withdrawal symptoms if present.

Hope you will find this answer helpful!

Feel free to ask me again, whenever you need!

Kind regards,

Dr. Iliri

Thank you for your answer. You are most helpful. Your answer was clear and helpful. When I get to one Percocet a day, how long will it take to get the oxycodone pretty much out of my system?

I would explain as follows:

Detailed Answer:
Hello again!

You get cleaned up from oxycodone as soon as you stop it.

It is not bonded to your proteins and it is not deposited in your tissues.

The most important matter is the changes in the cell receptors, which have lead to tolerance.

But, if you reduce the dose gradually, this problem is gradually repaired ( the receptors make a down regulation).

That is why I recommend this schema.

Everything will be OK before your surgery!

Hope you will find this answer helpful!

Wishing all the best,
