What Do My Lab Test Results Indicate?

default
Posted on Mon, 2 May 2016
Question: just went to the doctor and he explained the actual report to me. The wbc's were 6-10. The ref range is 0-5
No bacteria, no rbc's, no nitrates. Color was clear, straw. Specific gravity was at the low end of normal so urine was fairly diluted. Would that dilution mean the wbc's were higher if it hadn't been so diluted? My doctor thinks it's just the inflammation and no infection. He said we could repeatin a week or two and of it doesn't get worse then forget it. I do have irritation around the urethra and outer vaginal area which the gyn confirmed. Would that cause some wbc's from that irritation locally to get in the urine. My doctor said that makes sense but he wasn't sure, that only a urologist could answer that for sure. Also there was a small amount of yeast. He didn't care about that either. What do you think of all that?

This was a question asked in a thread with Dr Mangat but the questions were not answered before the discussion was closed.

If this is not Dr Mangat then you may ask whatever you need for clarification

Okay, I really was hoping for the answers to these questions specially asked.
My wbc's were 6-10 with ref range being 6-10. My doctor felt this is from irritation and not infection since the bacteria said negative (which is also why he said there was no reason to culture) Do you agree?
Could the outer vagina
Enrico and urethral irritation I feel (and which was confirmed
upon exam by gyn P. A.) cause the wbc finding?
Would the results be reliable even though the sample was fairly diluted (low normal specific gravity and straw color)?
Is a finding of small amount of yeast significant?
Should I use an over the counter vaginal yeast cream? Monostat?

Again, correction on wbc lab ref range... Should read 0-5

Sorry for auto correct. Should read, could urethral and outer vaginal irritation cause....

Brief Answer:
Yeast infections are uncommon.

Detailed Answer:
Hello XXXXXXX

You're advised not to use OTC drugs, like yeast creams.
wbc's suggest inflammation only.
In severe cases, urine nitrites will be positive.
Also, urine culture will be positive for bacterial growth, but not indicated in all.

Your reports are negative for nitrates.
As you're in good health and asymptomatic, with negative urine nitrates, nothing else was advised.

So again, my advice to you remains, relax and enjoy life.

Nor totally asymptomatic as I stayed. Irritation remains...and as stated in previous questions... I didn't have a culture but the microscopic exam was neg for bacteria which is why doctor said culture was pointless.
Would the urethral and vaginal irritation account for wbc and is the small amt of yeast a concern?
Is fairly diluted urine still accurate for results?
doctor
Answered by Dr. Matthew J. Mangat (15 minutes later)
Brief Answer:
Minimal irritation should be negligible.

Detailed Answer:
Hi XXXXXXX

Minimal irritation of the genitalia area, isn't a cause for concern.
On examination, the gynecologist also didn't find anything significant.
Then what are you worried about.

Yeast infections are really uncommon, so don't worry about it either.

Increased fluid intake will lead to diluted urine and will appear as pale urine.
Decreased fluid intake, will cause concentrated urine, appearing dark yellow.

Hope your doubts are cleared.
Maintain 8-10 glasses fluids daily.
If you sweat more while exercising, increase according to amount of sweat.

Wishing you good health.

I know what causes diluted, pale urine. I what I wanted to know is if the results are still accurate in diluted urine?
I also wanted to know, would urethral (causing some discomfort on urination) and vaginal irritation account for slightly elevated white count?
I also wanted to know if finding of yeast is important and would it be coming from vaginal?
I also wanted to know if there was infection needing to culture is my doctor correct that the microscopic exam would have shown positive for bacteria? (I have had infections with neg nitrites}
I would appreciate specific answers to those 4 questions. Thank you

Brief Answer:
All doubts cleared.

Detailed Answer:
Hi XXXXXXX

1.The microscopy urine test will not be affected by dilute urine.

2. Inflammation can cause wbc's in urine.

3. Yeast isn't considered of significance, in general.

4.Let's not consider imaginary situations now.

Wish you well.

So, to clarify, inflammation specifically just of urethra and external vaginal tissue can cause wbc's?
Would bacteria have been present on microscopic if there was infection needing to be cultured?
Since I've been on antibiotics, would it be reasonable to consider a vaginal yeast infection as reason for yeast, and possibly even wbc's?

Brief Answer:
wbc's i urine, seen mainly with inflammation, mainly urethra.

Detailed Answer:
Hi XXXXXXX

My impression is,you're going to be able to advise people having your symptoms, just like an Urologist does. Excellent questions asked so far.

To be in perspective, the entire lining of the urinary tract,is the same.
So, inflammation may extend anywhere, along the urinary tract.
Usually the implication in people having your symptoms,is the urethra and genitalia.

Restrict your self to factual situations, not hypothetical one's.
Bacteria is seen being suspected here.

Why are you chasing a few yeasts.?
I've mentioned it's uncommon.
You may do an urine routine and culture after one month,to clear your doubts.

Have a relaxed lifestyle.
Hope you don't neglect your daily exercises.

Did you mean to say, "bacteria is seen being suspected here"? That sounds like bacteria is suspected in this case when I thought you and my doctor said no because none found in microscopic. Did I misunderstand you?

Actually, in my experience and of those around me, yeast infections, especially after antibiotics, are quite common. Vaginal ones I mean.

Would u do a culture in one month if no bacteria on microscopic as now? Because my doctor, and I thought you, said no need if no bacteria on microscopic

Brief Answer:
bacterial infection's not suspected.

Detailed Answer:
Hello XXXXXXX

Forget the bacteria.
There's very minimal inflammation, as seen in your reports,of few wbc's.

After a gynecologists' examination, she never suspected yeast infection.
So, you can forget the yeast.

If you read my previous reply again,i've mentioned, it may be done to clear your doubt only. Not that it must be done.

Hope this clears your doubts.

Actually, the gyn P. A. Did think the irritation looked somewhat like yeast and sent out a culture. It came back negative. However tthat was 3 weeks ago, while still on antibiotics so I was thinking perhaps it was just starting and, as monostat is easily available in the US, over the counter as a vaginal/topical cream, and is inexpensive, what would be the harm of seeing if this helps as there was some yeast which could easily come from vaginal tissues. I've used it many times in my lifetime.

Brief Answer:
Never take Non prescription drugs.

Detailed Answer:
Hello XXXXXXX

If you ask me, as an Urologist,we never recommend unscientific treatment.
Just because an ointment or cream is easily available, you should not be using it.

Especially in your case, when yeast culture was done, which you never revealed till now,and it has been negative, why are you worried about it.

Wish you well.

Because it was negative before finishing antibiotics and it's been 3 weeks and there was yeast in urine. Monostat used to be by prescription, now it isn't.

I'm not worried about having a vaginal yeast infection. It's a pretty simple answer and not scary. Kind of hoped it would help.

Is some yeast a common finding esp in women?

I know this sounds ridiculous but I've been urinating in a glass jar to see if it looks clear it has been. However, this morning it was clear and only a little while later, it looks cloudy. And often it has a few little flakes. Now I'm panicking again.

Okay... Drank alot of water with dandelion herb teabag in it. Now urine is clear again. Don't know why it wasn't earlier... Maybe I was wrong or not quite as hydrated?? There was still few small flakes but when I looked, I notice there are several flakes all around the vaginal and urethra area. Toilet paper fragment or dried estriol cream... Which I can see in the folds. Geez I bet no one examines things like I do!

My doctor says I need to go to a urologist if I get another infection as he feels it's out of his expertise to treat properly. What would a urologist do or suspect? That kind of worries me. Actually, who am I kidding, it worries me alot! I almost think I fear another infection so much that I almost bring it on and I'm sure the stress aggravates the nerves that make me feel as if I need to urinate more etc. And concentrating on it also magnifies the symptoms, of that I'm sure.
