What Do My Lab Test Reports Indicate?

Question: Dear Doctor,

With regard to the preventive health check up reports, out of 84 tests, in 16 tests the values
do not tally with the reference range. However, as I am health and so far no health issues
or symptoms, I am concerned with regard to values for

1. High Sensitivity C-Reactive Protein which is 5.31 against normal value of < 1 for low risk
and much higher than the the level of >3 for high risk category.

Can results go wrong when done on automated high tech instruments? At no time in the
past I had any issues connected with heart nor was it diagnosed during medical check ups.

2. 25-OH Vit D total value is <4.2 as against 3 - 100 to indicate sufficiency.

3. Uric acid is 1.82 vs. 3.5 to 7.2

4. Blood Urea Nitrogen is 5.26 vs 7.9 to 20

So also few more results are in variance with the normal range.

Except that I am on anti epileptic medication for the last 7 years and also seizure free for the last four years,
I have nothing else to add.

How would you guide me in the present context? Do I need a re-do of the tests or do I take some medication
and see the results over a period? Please guide me.

Thanks for sending the reports.

I can see that some of the values are out of range for this laboratory. But if you check these values against international reference range then you will observe that they are just outside the range.

Especially the CRP value, it is only 5.31. CRP is a non-specific test that is raised in inflammation, especially of the joints. If you have some joint pain then it may be of value otherwise it can be ignored. It is also related to cardiovascular disease etc. But it is not an alarming value. It means that you should regularly monitor your blood pressure.

The electrolyes are low which may be due to dehydration like sweating in hot weather, etc.

Otherwise your reports are normal. You can repeat the tests after 1-2 years. No need for any medicine unless some medical condition is diagnosed.

Your reply has made me understand the values of the tests in proper perspective.
I would be satisfied if you can throw light on my following queries.

1. How do you see hsCRP value in co-relation with values for Liver Profile, specially GGT and Alkaline Phosphatase?

2. Do I need to see any specialist or do I need to have any other tests done to rule out any problem?

3. Can I continue with my gym?

4. Anything you see in the test results that suggest Coronary Calcium tested to rule out any CVD issue?

5. My Vit D being very low, do you suggest any supplements? My exposure to sun is limited as I am out for work between 9 & 6 and that I travel by auto. Is it ok that Saturdays and Sundays, if I expose my self to sun in the mornings and evenings by walk, will it help?

6. I would like to hear your views, if possible, re. low values for Uric Acid and BUN.
If you don't feel that I am burdening you with more queries, I would welcome the same as it will give me a better sense of direction.

I will answer your queries specifically.

A. Yes both are high. It means that the arteries are depositing fat and plaque. But still the values are low.

2. Do I need to see any specialist or do I need to have any other tests done to rule out any problem?
A. I dont see a need to see a specialist.

3. Can I continue with my gym?
A. Yes you can continue with your gym

4. Anything you see in the test results that suggest Coronary Calcium tested to rule out any CVD issue?
A. None

A. It will help to walk in morning and evening. But the vit D values are low and you will need supplements. You can take keycal d3 or divion XXXXXXX (60,000 IU) once every two weeks for 1 month and then once a month for 3 months. Keep following up with your doctor about this every 6 months.

6. I would like to hear your views, if possible, re. low values for Uric Acid and BUN.
A. low levels of uric acid and BUN are not of any concern. Low uric acid, BUN, electrolytes may be due to drinking excessive water. I would suggest that you drink only that much water that you are feeling thirsty for. Also while exercising drink water and electrolytes (for example in gatorade). Take adequate diet i.e 3 proper meals and 3-4 snacks in a day.

As I got some thoughts in the mean time, I thought I should share with you as this is a platform where you feel encouraged to get all doubts cleared from dedicated professionals.

1. My check up has revealed, as you have noticed, results on two extreams - Some parameters on the higher side (like hs CRP in Cardiac Risk Marker) and some on the lower side (like Lipic Profile and Renal Profile) of the normal range. Is it unusual?

2. I do not have any joint pain or any inflammatory condition except that for over a month prior to my preventive health check up, I had severest running nose that was attributed to allergy and possible sinusitis which was later ruled out. I am on healthy veg diet avoiding fried items. My BP is generally normal. I will keep a tab on this. In view of this how I can improve upon my hsCRP over the next six months before I have a recheck.

Traditionally I prefer to drink ginger tea and also take turmeric along with milk in the night to fight inflammation. I drink about 5 litres of water everyday. May be I can reduce it 2-3 litres based on the thirst need.

I value and appreciate your views based on practical experience.

Hi again

It is not unusual to have such results. Drinking excessive water has caused lowering of lipid profile, renal profile, electrolytes. Your water intake has to be driven by thirst only. Do you have swelling or fluid retention in body?

High hsCRP is usually seen if conditions like diabetes, high blood pressure, joint pain, etc. All these conditions are associated with inflammation and that is why the increased CRP values. Lowering the inflammation will lower the crp but its not that easy. Some ways to reduce inflammations is to take hygienic healthy food, cleaner water, take becosule capsules, avoid unnecessary medicines, do not take fish or fish oil, exercise daily, etc.
