Question: Dear Doctor,
I am 78, with Type II Diabetes on insulin. In routine examination for other ailment it was notice that I have a Grade I Fatty liver with SGPT 353 and SGOT 363 (IFCC Modified method). I am a non alcoholic. All other parameters, like Bilirubin, Alkaline phosphatase and Creatinine etc.) in Liver Function Test are with in the reference range.
Kindly suggest the line of treatment.

I have gone through the mentioned details. The grade I fatty liver seems to be in direct association with your diabetes and non-alcoholic fatty liver disease is a known entity. I would need to ask specifically how long you have been diabetic, the level of control, whether you are on regular medication and whether you are obese.

Not much can be done about it right now. Treatment is linked to the management of the diabetes and obesity. You may take Vitamin E 400 IU capsules once or twice daily. Take curd or yogurt on regular basis since it contains beneficial bacteria. Alternatively, pro-biotics/pre-biotics may be taken. Discuss with your doctor about use of Pentoxifylline, an anti-cytokine.

Dear Dr. Saha,

Thank you for your reply. I have been a diabetic for the last 16 years. The sugar levels are monitored regularly and kept under control with in the limits as far as possible. I am not obese. I am about 70 Kg with an height of 5'-3'', which is slightly on the higher side, but this weight is being maintained for last 40 years.

I would like to know whether Endocrinologist will deal with my case or a gastroenterologist?

You can take an opinion from a gastroenterologist.

Thanks for writing back. It is good to know that you are non-obese and that your diabetes is under control. In such a scenario the scope and significance of intervention is very limited. However, you can get an opinion from a gastroenterologist although your endocrinologist would be able to handle such a presentation.

Not much suggestions to add. Just follow a healthy lifestyle. Exercise regularly. That is all you need to do.
