What Do Hardness On The Penis Tip And Pain During Urination Indicate?

Question: I have a hardness on the end of my penis. I am experiencing some pain when I urinate. I have some pain when I press on this spot. I have already had a radical prostatectomy 20 years ago.

1. Do you feel a bump/lump/boil at the tip of your penis?

2. Do you notice a redness or change in colour and/or external appearance of the hardened region of the penis?

3. How would you rate your general hygiene on a scale of 0 to 5, 0 being poor and 5 being paranoid

4. Have you been diagnosed with any chronic conditions like diabetes, hypertension, etc?

5. Have you been diagnosed with a urinary tract infections in the past?

6. Do you have any kidney stones?

7. Any urinary incontinence issues?

8. Any symptoms like burning sensation while urinating, increased frequency of urination, increased urge to urinate but very less urine passing, etc.?

1. It feels like a small bump toward the tip of the penis.
2. There appears to be some redness in the area from what I can see.
3. I rate my cleanliness at 4.
4. I have high blood pressure that is treated with Micardis. It is in the normal range when I see my doctor or other doctors. I am not a diabetic.
5. I have had one UTI about 7 years ago.
6. I do not have kidney stones right now. I have had them in the past and know what pain goes with kidney stones.
7. I am not experiencing any urinary incontinence.
8. There is a slight burning sensation when I urinate. The frequency and urges are normal.

Could be a simple bump requiring warm compress, but better to examine

Hello once again and thank you for that sir.

Well it appears the bump could be the cause for your discomfort and will have to be examined by a doctor sir. In my opinion, it could be just a simple bump that should subside on itself in time, but:

1. The symptoms and discomfort you are experiencing could result in a UTI and so antibiotic coverage early during the course of the infection can help save you from a lot of distressing symptoms.

2. What if the bump is not a simple self-resolving one, and should have been treated for what it is early on in its course. I do not wish to alarm you, but only wish for you to be safer than sorry.

So if you can afford it, and especially if you have insurance it would be most wise to visit your doctor for an elaborate evaluation into the bump; once all other conditions are ruled out; simple antibiotics, pain management medication and warm sitz baths along with warm compresses indirectly over the region should help solve the issue.

The situation resolved itself this morning. My urine stream was not as strong as usual and did not cut stop when I tried to stop. It continued to flow. This morning I exerted more pressure while urinating and a kidney stone popped out.
I guess that explains the bump, the discomfort, and the change in the urine stream. I know a kidney stone because I have seen them at least three times before. What was confounding is that I did not experience any pain in the kidneys beforehand.
The question now is whether there is a need for followup with a urologist.

Hello once again sir.

That is good news, which is why I asked you in my previous response about kidney stones.

But it is great to know you are relieved now.

I do not see any reason why you need to visit a urologist now, but I do think you need to do 2 things:

1. Increase your fluid intake (preferably water) to about 2-3 litres a day for the rest of your life

2. Schedule an ultrasound KUB (kidney, ureter, bladder) once every 6 months to check if there are any stones in your renal tract or if they have changed in size, etc.
