What Do Delayed Periods And Headaches Indicate?

Question: Hi, a cousin of mine aged 22 yrs was suffering from delayed periods and head ache since 2-3 yrs. She had recently taken an USG abdomen which revealed as Spleenomegaly, Large PCOD, Mild pelvic inflammation. When she had taken an usg abdomen 7 months back, the spleen was "enlarged at 7.5 cm" . However in the current USG it shows that the spleen is enlarged as 12.5 cm. Can you pls suggest on this. Is this something fatal? should we undergo more tests

- lymphoma - leukemia - hemolytic anemia - tumors of spleen or other organs
- viral , bacterial , parasitic infection - inflammatory conditions as lupus ,
rheumatoid - others .
* This is not fatal if taken care properly and evaluated properly .

HI,

She complains of head ache and irregular periods. Her periods is mostly delayed. She don't have complaints of Joint pain, fever, dark urine , rash etc.. However, she is complaints of easy fatigue.

No H/o weight loss.

* Irregular periods and PCOS must be best managed by gynecologist .

* For Spleen enlargement , I strongly recommend to consult an expert
Hematologist & get further evaluations which include various parameters as

- Complete blood counts including hemoglobin , white , red blood cell ,
reticulocyte , platelet , neutrophils , lymphocyte counts .
- Smear studies for malaria .
- Coombs test for G6PD deficiency .
- Kala azar evaluation in microscopy .
- Serum IgM levels .
- Serology for viruses .
- Core needle biopsy ( Ultrasound guided ) in selected cases .

* These are certain evaluations as per the case narration , however complete &
perfect list can only be possible after clinical examination .

* Need of advance secondary level diagnostic requirements depend upon results
of the above .
