Brief Answer:
You need to be re-examined by your doctor

Detailed Answer:
Hello,

the vision blurring is not a good sign. It might mean significant expansion of the infection to the sinuses. Amoxicillin/Clavulanate 875/125 is a potent first choice antibiotic for both the ears and the sinuses but if your doctor believes that it's not effective enough, there are second line choices like levofloxacin or azithromycin. The choice may depend on the assessment of the infection expansion. Azithromycin would be better as a second line agent for the ear. Levofloxacin would more effectively treat a sinus infection.

In cases like this a decongestant may also help because it may relieve the swelling of the eustachian tubes and the swelling of the nasal passages.
