Brief Answer:
Seems to be vestibular migraine, vestibular meniere's or FEA Type 2.

Detailed Answer:
Hi,

Thank you for your query.

1. Your history of having to retire to a dark quiet room suggests a migraine associated vertigo. The migraine will be masked by beta blockers which may have been started for some other medical problem. Vestibular Meneiere's and Familial Episodic Ataxia (FEA) Type 2 are other possibilities.

2. You may also have other conditions such as motion sickness where retiring to a dark quiet room decreases the visual mismatch and hence makes you feel better.

3. What is the dose of betahistine (Serc) that you are taking? Upload detailed reports for review. I would need to review the exact details of the dizziness, the medications tried earlier (specifically migraine medication, acetazolamide and topiramate) and investigations, clinical notes.

4. Symptoms such as loss of smell, taste maybe explained by the migraine associated vertigo or as side effects of medication such as Lovan (fluoxetine).

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
