Question: For a male friend: Swollen left hand with redness on the surface. Could it be heart related? Painful as well with loss of use.

No, it is not heart related. Heart related issue would cause cold and pale hand.
In his case it may be caused by veins thrombosis or soft tissues inflammation.
It would bd good to have it checked with Dopplers ultrasound. He should consult a doctor.
