Question: My daughter is 7.5 years old, She is having hard time in walking from last 1.5month, she is having hard time in sitting in car, coming out of car, sitting on her feet down, bending knees and complaining of pain occasionally . Before 2 months she was very active, dancing, jumping but she fall down in school 2months back. After that she is not able to walk properly, she walks but with effort which can be noticed. We told doctor and her xray was done of feet and was declared she is flat foot, and nothing came in x-ray, but pain gets worse and ambulation doesnot get better. Than we took her to orthopedriatric , they asked us to do labs of all her blood counts, Her ESR is 36 , rest all blood labs are normal. We are worried that is she having juvenile arthritis, SLE, or any other arthritis, in so much stress or this is something which happen after her fall , or something else.

Brief Answer:
The exact location of problem needs to be identified first.

Detailed Answer:
Thanks for asking on HealthcareMagic.

I have gone through the details and understand your concerns. Let me approach the problem step by step. The first step is to identify whether the problem is involving the joints or whether it is involving the muscles in between. If the problem involves the joints, the orthopedician or more importantly, a rheumatolgist would be be able to look into the problem. But if the problem involves muscular weakness, the problem could actually be either in the muscles (myopathy), neuromuscular junction, or the end-nerves or motor neurons supplying the muscles. A neurologist can evaluate your child and tell you the exact focus of the problem. You can proceed accordingly thereafter.

I would not worry about the ESR. It is not very high and is a very non-specific marker.

Feel free to ask me back in case of further queries.

Regards
