Question: Hi Dr hope u know my sin 4.3 years with adenoiditis.levocitriz mont tablets for month.
W used metaspray from last Feb 2014 to April 14 for 12 weeks.he was ok.but again in augest once he got adenoid infection and 2nd time November all treated with antibiotics. He was OK with this.now from 2days he is having severe snoring and mouth breathing.so Dr advised me
1.hicet 3ml twice for 5 days
2.levocetrizine tablets 30 for month at 6pm once
3.duonase nasal spray at night.
As this we see last and if not feeling recovery we plan to go adenoidectomy.
My query is hicet is citrizine and the tablet given also levocitrizin will it be overdose of citrizen?

Is metaspay and duonase spary same or different?

Brief Answer:
The medicines are similar. Do not use them together.

Detailed Answer:
Hi,

Thank you for your query.

1. It is normal for a child to get 3-4 episodes of upper respiratory tract infections in a year. These can be easily managed with medication.

2. Yes, it is better to give only cetrizine or levocetrizine, not both together as this may lead to an overdose.

3. Duonase is a combination of azelastine and fluticasone, while Metaspray contains mometasone. They are also similar. Metaspray is better as the absorption into the body is lower. These sprays should be temporarily discontinued during an acute infection.

4. There is no need to rush into surgery (adenoidectomy) for such brief episodes.

5. Upload any adenoid X-rays that he may have undergone.

If you have any more questions I will be available to answer them.

Regards.

Brief Answer:
Continue the current treatment.

Detailed Answer:
Hi,

Thank you for writing back.

1. Yes, from the X-ray image, he does have adenoid hypertrophy.

2. Your current treatment plan is fine.

3. Adenoid tissue enlarges normally between the ages of 3 and 7 years. Thereafter it decreases in size on its own and usually disappears by the age of 12 to 15 years.

4. Adenoidectomy needs to be done only if the symptoms are severe or there are other symptoms and signs (collectively known as 'Adenoid Facies'). Let your lead physician decide the further course of action.

If you have any more questions I will be available to answer them.

Regards.
