What Causes Severe Tooth Pain Post A Root Canal?

Question: Severe tooth pain 13-16 had drilling and fillings of 13-15. Pain became worse. Had root canal on 13 2 days later pain was excruciating tooth was fractured and extracted. 5 days later pain became worse wisdom tooth full of decay was extracted. No change. Radiographic studies including CT were done no evidence of osteomyelitis sinusitis or abscess formation. Oral surgeon reccomended extraction of 14 & 15 with susbsequent implants after healing in 13-15. This was done 2 days ago and pain remains with radiation up to left nostril. Quality is pressure like with some burning and absence of sharp knifelike pain that was initial complaint. Oral surgeon reccomended prednisone taper and to continue antibiotics for 3 more days. Please help in telling me what u think is cause of pain starting to affect my life been going on for a month- thxs XXXXXXX

I have gone through your query and can understand your concerns..

As per your complain it seems that you have got extraction of left upper wisdom tooth that is tooth no.16.
Along with it you have also got extraction of 13,14 and 15 no teeth that is second premolar and both the molar teeth..
And presently you do not have knife-like pain and the present status is pressure and burning feeling..

As per the entire sequence of pain and treatment as well as the medications given to you, it shows that as you have multiple teeth that were infected and extraction was done, there has been excessive inflammation..
When the infection is severe and especially involving the larger area, the infection and pain can take a longer time to resolve..

Other thing is that pain from one or more teeth can radiate to the entire affected side and you can feel pain in multiple teeth even the one that are not infected..

In your case also it seems the same and pain in nostril seems to be radiating pain from extraction sites..

Nothing to worry, continue taking the medication as advised to you..
Prednisolone is a steroid and will help in reducing inflammation and also pain will reduce..
Antibiotics will help in reducing Infection..

You can also take anti-inflammatory painkiller like Ibuprofen..
Do cool compresses over the cheek as well as nose..
You can apply numbing gel containing Lignocaine or Choline Salicylate over the gums of the extraction sockets to relieve pain..
Take a soft diet with more of fluids, but avoid taking hot and spicy foods as it can add up to irritation..
You can also do cool water rinses but avoid doing vigorous gargling..
Maintain a good oral hygiene as it will help in faster recovery..

Thank you i was prescribed the prednisone for 6 days and the cefuroxime for 10 days prior to this i was on augmentin for 4 days but difficult to tolerate. My concern was i had some kind of nerve damage when the radiologic studies were normal should i be concerned about this or just treat as above and your fine recomendations. Also if i understand correctly the multiple extractions and prior procedures withinin a few weeks could also add to increased recovery time is that correct- with regards XXXXXXX

As per your query the symptoms does not look like that of nerve damage and it is most probably due to infection and inflammation only..

As your radiological investigations also does not reveal any abnormality, you need not to worry.

Other thing is that, as there has been multiple teeth extractions, the healing will take place gradually.
