Brief Answer:
Varicocele/ epididymo-orchitis may be the reason

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/58 - balls hurt when walking - seems to be getting worse - almost unbearable at times - not visited a Doctor - medications tried : nothing...

From the short history you have given, the probable reasons can be:
Varicocele
Epididymo-orchitis
Hernia with neurogenic pains

This can be diagnosed by:
Ultrasound of the scrotum with color doppler examination and ultrasound of the whole abdomen, particularly genito-urinary.
Tests of blood and urine.
Clinical evaluation, actual examination and correlation of the clinical diagnosis with the reports to get an exact diagnosis hence allowing accurate curative treatment.
Till then use scrotal support to elevate the testicles and keep supported.

I hope this answer helps you to get a proper diagnosis and treatment.
