What Causes Severe Headache When Under Stress?

Question: Hi Doctor
I am 30 years old male with 80 kg weight. From Last 3 years i am going through severe tension and stress. Sometimes i sweat when stress level is high. Currently my BP is 130/80. Generally when i am in stress i am having severe headache in regular interval Both Front & backside of the head i.e in the forehead near the eyebrows & near the back neck nerve i am having vigorous pain since few months. Also the pain has increased towards my right hand arms from few days. Sometimes i am having right side chest pain may be due to gastric. Please suggest what may be the reason for the above symptoms? and what sort of action i should take? Is this a symptom of particular kind of headache?

Probably Tension headache,Cardiac evaluation is needed.

Detailed Answer:
Hi Sir,

I understand your query and concern.

Your symptoms are suggestive of Tension headache.

This type of headache has atypical presentations as seen in your case.

Chest pain radiating to arm on and off should be evaluated for any ongoing ischemia of your heart.

So I advise you to get an ECG,2 Dimensional echocardiography of your heart,lipid profile.

Meanwhile analgesics like Tramadol and antacids like Ulpan anti anxiety drugs like clonazepam will help.

Note:These drugs should be taken under strict prescription of your doctor.

Avoid stress and anxiety.

Regular exercise for 30 min,low fat salt restricted diet to be taken as your blood pressure is under high normal note.

Monitor your Blood pressure regularly.

hi doctor

i never had high or low BP .but when i checked its 130/80 . is it normal or is it high BP? i have checked on internet i found out if the headache along with pain in back of the neck MRI is needed for correct analysis but my headache occors when i am in stress and it rises till back of the neck . is this stress related headache or i need to do MRI test. is it serious concern which i shouldnt neglect . my wife also getting the same headache with pain in back of the head when she is in stress . so what do you suggest?

MRI not needed as of now.

Hi Sir,

Welcome again.

Your symptoms are definitely suggestive of Tension headache,so you may not need an MRI.

So I advise you to start the drugs which I mentioned earlier.

You need MRI only if your symptoms does not relieve even after taking standard above mentioned drug therapy for 2-3 weeks.

You (and your wife) also need to go for stress management like yoga, exercise immediately as I said earlier.

Coming to your blood pressure 130/80 mmhg is considered high normal at your age. So I advise you to monitor regularly for every 2 weeks.

Try to maintain your blood pressure between 110/80 -129/80 mmhg which is normal at your age.
