What Causes Severe Back Pain With Normal CT Scan?
Question: Hi. I have severe back pain around left kidney area. Went to emergency yesterday. They thought it was kidney stones but CT scan came back negative;however doctor noticed a small hernia and said that was the cause of pain. They sent me home with tramadol and an anti inflammatory med. they set up appt for me with hernia doctor But pain almost constant and a little worse

Normal CT would have conclusively ruled out any significant problem. Hernia if it's reducible is unlikely to cause any pain.
It would be very helpful if you could provide me with the following additional information
1 Where is exact location of pain
2. Do you have associated constipation
3. Where is the location of Hernia ?is it in inguinal or lumbar (flank region )
4. Do you have any urinary complaints

No urinary complaints. Yes, constipated. Hernia is in front under belly towards left side. Dr. Said hernia is surrounded by fat and air. Pain is in back on my back and travels to front under left ribs.

The pain is likley to be due to
1. Ischemia of fat ( When the fat protrudes in case of hernia there is constriction at opening of hernial sac causing reduced blood supply and ischemia)
2. Associated constipation
If the hernia is clinically completely reducible( there is no obvious swelling at hernial site on manually reducing it), there is no cause for concern.
I would suggest you the following
1. Ensure adquate intake of water: 2-3 litre a day. It will take care of constipation
2. High fibre diet: 2 servings of vegetable salad and 2 servings of fruits especially citrus fruits will ensure intake of fibre
3. Laxatives like lactulose will soften the stools
Do consult your Physician if the hernia does not get reduced manually.
