What Causes Right-sided Flank Pain, Nausea, Severe Diarrhea, Vomiting And Dehydration?

Question: I have had right flank pain ranging from 3 to 7 on a 0-10 pain scale (0 being no pain and 10 being agonizing pain), accompanied by mild intermittent nausea, and moderately increased frequency and volume of urine (normal color), for about two weeks. Started in lower right quadrant and migrated up to upper right quadrant and general kidney area (now involves upper and lower right quadrant). It felt like menstrual cramps and soreness in the psoas during the first week or so. It moderately impairs my normal daily activities. No fever, but I don't have a thermometer so I can't really swear to that. This came on shortly (2-4 days) after a severe allergic reaction to food causing about 40 hours of diarrhea and intermittent vomiting and severe dehydration secondary to diarrhea and vomiting. What should I do that won't bankrupt me?

As per your description, it looks like residual infection of the Gastrointestinal tract. Diarrhoea was due to ingestion of contaminated food.It occurs when the colon is involved. Caecum and Ascending colon are present on the right side of the abdomen. Infection and inflammation of these organs will lead to diarrhoea. In your case,the infection is not totally controlled and the residual disease might be responsible for the present symptoms.
I prescribe Antibiotic Levofloxacin 250mg twice daily for 5 days, a Pre and probiotic capsule twice daily for 5 days along with anti spasmodic tablet containing Dicyclomine twice daily for 5 days to my patients with similar complaints.
If there is no response, I will go for Ultrasound abdomen to rule out Right sided kidney or Ureteric stone.
