Posted on Tue, 7 Apr 2015
Question: GOOD EVENING

My nephew (21 yrs old) has had a pain in his toe for months now and puss would come out of it. This a cause him severe pain in the toes. Mt sister took him to the doctor and they told her there was nothing wrong. This Saturday my sister squeezed the toe to get all the puss out and a bean size white hard puss flew out of my nephews toe. He refuses to go to the ER to see what that bean size object was. Any idea what that could be?

Brief Answer:
the tissue is pyogenic membrane

Detailed Answer:
Hi
Welcome to Healthcare-Magic
Greetings of the day

Dear Ms

Thanks for providing clinical photograph. The white membrane like thing which came out resembling white bean is pyogenic membrane. When there is formation of pus ,it is surrounded by membrane known as pyogenic membrane. over period of time the pus may itself solidify and lead to thickened firm bean like structure. Other possibility that needs to be considered is foreign body granuloma.
As the pus and membrane has been squeezed out and if there is no associated redness and pain then there seems to be no cause for concern. Do visit physician for routine assessment. The bean like structure removed can be preserved in small formalin bottle for further assessment.
The present wound also looks fine with healthy granulation tissue. Local application of hydrocolloid dressing will help in wound healing.

Do get back to me if you need any further assistance, will be glad to assist you.

Wishing you and your family a very happy and healthy life.

Take care
