Question: Hi, my name is XXXXXXX I have been having chest pain for about three months, I am 53 years old and weigh 170 lbs, I have all the tests including an angiagram which were all clear,I am on tablets to lower blood pressure (which was normal ), lower cholesterol ( 5.1), and a gastro tablet. I continue to have this pain daily from chest to arms lasting approx. 2 min at 10 min intervals for up to 3 hrs. Any ideas ".. Regards XXXXXXX

1) Nature of chest pain, location, exaggerating, relieving factor, intensity on grade of 1 to 10. Is the pain related to meals?

2) Associated breathlessness, leg swelling.

3) If there is a history of trauma to the chest?

4) Past medical history with all medicines you are taking currently?

5) If any 2d echo/ angiography report is available? Lipid profile reports if available?

These details are required to know if you have a cardiac or non cardiac pain?

1) chest center to arms painful and sore muscles,rate 9, sitting up straight shallow breathing for 2 min relief,not related to meals.
2) no breathlessness.
3) involved in car crash 5 years ago, seatbelt injuries nothing serious.
4) one gastro tablet, one staten cholesterol (5.1), one aspirin
(precautionary).
5) 2d echo clear, angiogram clear, blood pressure normal, non cardiac.

I have read your response to my questions and the echo and angiogram reports. I hope these are pretty recent.

Given the description of pain, it clinically seems muscular in origin. Also normal angiogram rules out an ischemic heart disease.

Another uncommon presentation would be vitamin d deficiency. I have seen some patients present with chest pain and they responded to vitamin D. I would suggest you to have vitamin D 3 levels done. also a fresh EKG would be helpful to rule out any new cardiac event.

For symptomatic relief I would recommend you a pain reliever and a muscle relaxant.
