What Causes Pressure Inside My Head While Sitting And Moving Around?

Question: Hello, lately I have been feeling a general sensation of pressure inside my head while sitting, standing, or moving around, which can turn into swirling sensations inside my head while lying down. It does not feel like the room, or the world is spinning or swirling, just inside my head. Other times while lying down I am feeling a surging, almost electrical, sensation inside my head that ebbs and flows. It doesn't actually hurt per se, but the sensation is uncomfortable---and scary---enough that it kept me awake for several hours last night.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your medical history.Thanks for being elaborate.
If you would have presented to me as an outpatient,I would have put the following differential diagnosis for your symptoms-
1)Posture or occupation related-If you are sitting in front of the computer screen for long hours or otherwise have some job sitting for long hours,then these symptoms may be a part of posture related problems.If this is the case,please meet a physiotherapist and he will be able to help you in postural adjustments.

2)Anxiety-Anxiety can have varied manifestations and this can be a part of anxiety.If you are anxious enough,please start doing mental relaxation exercises/yoga/meditation and anti-anxiety medicines may help in severe cases.

3)Stress headache or Tension type headache(TTH) classically presents with pressure in head.Swirling and spinning sensations are not uncommon.Drug of choice is amitriptyline in addition to the mental relaxation/Yoga.

4)Sinusitis/Ear pathology-These can result in such symptoms and ENT consultation should be sought.

5)Cervicogenic-X-ray cervical spine should be done as a part of evaluation as cervical spondylosis can result in such manifestations.some of the vertebral problems and discitis may also result in such symptoms.

6)Ophthalmology opinion should be taken to rule out 'papilledema' due to raised intracranial tension.If papilledema is detected ,brain imaging like CT or MRI Brain is indicated.

Papilledema if detected can be serious and should be urgently evaluated by a Brain MRI or CT.
