Question: Hi,
I have a tooth abcess. today my dentist drilled a hole in my tooth and drained it. I have swelling under my jaw and under my chin that is very painful and it is hard to swallow. The swelling is sort of firm to touch. I am taking pen v-k., ibuphrophen and vicodin. can I wait till the morning to evaluate my swelling or should I be concerned now.

I have read your query , reviewed your medications and understand your concern.

In my opinion , you should wait till morning to evaluate swelling under jaw & chin . If it is not subsiding with medications within 48 hours then you should be concern about it .It is common to have swelling under jaw & chin after tooth drilling for tooth abscess .

If I were your treating Doctor for this case , I would advice
use antiseptic Mouth Wash twice daily like chlorhexidine or betadin &
continue pen v-k., ibuprofen and vicodin .

Severe headache & jaw pain will subside with medication . It is also common after tooth drilling .

Take semi-solid food till swelling subside .

You will get relief with medications. No need to concern now .

so this morning i woke up with more swelling under my neck and swelling under my tongue. hurts more to swallow.

Dr. Karade,
today ( 8/12 ) I had a root canal of the infected tooth. the cellulitis has gone from under my left mandible to under my jaw down my neck just below my thyroid. I am not having breathing difficulties. Just severe pain. I am almost at 48 hours of taking Pen V K. 500 mg 4 times a day. If my cellulitis gets worse, what would be an appropriate IV antibiotic for them to give me. Just would like 2 doctors opinions. You and my endodontist. Just want to make sure the choices of IV meds are the same.

I would advice do investigation like ultrasonography (USG) of neck to rule out infection & pus formation in the submandibular space (in your neck).

Antibiotics should be broad-spectrum and cover gram-positive, gram-negative, and anaerobic organisms (different kinds of bacteria that need different types of antibiotcs). Combinations of penicillin, clindamycin, and metronidazole are typically used. These are all prescription meds of course and in your case your doctor will provide the prescriptions and directions for use based on his/her physical examination of you.

Continue anti inflammatory ibuprofen , vicodin & Mouth Wash .
