Question: Dear Doc - I am a 41 year old male . I have a scalp related issue - I have many small boil types on my scalps . I dont have any dandruff issue (though I am balding) . Can you please recommend any OTC medicine or topical ointment to manage this . And it is painful :-( . I have used Nizoral (twice a week) and also switched to baby shampoo . Its bothering me a lot.

Brief Answer: Antiseborrheics+antifungals+antibacterials Detailed Answer: Hi. Scalp is a seborrheic area which is rich in oil glands. Malassezia yeast feeds on the secretion of the oil glands, Sometimes it can hyperproliferate thereby giving rise to itchy, painful bumps. There can be secondary bacterial infection of these bumps as well giving rise to pus and pain. Therefore I would advice that you avoid using any oil on your scalp and try to keep it as clean as possible. Nizoral contains 2% ketoconazole which is an effective anti-seborrheic agent. I would normally recommend usage of ketoconazole every alternate day in my patient. It can even be use daily or even twice daily according to the requirement of an individual patient. For the bumps I would recommend that you start with an oral antibacterial (penicillin group e.g Amoxycillin in combination with clavulanic acid) for a week. An oral antifungal e.g Itraconazole 100mg twice daily for a week is also advisable in your case. Hope this answers your query regards
