Brief Answer:
To find the cause and treat it . .. .

Detailed Answer:
Hi.
Thanks for your query.

Male / 52 - Circumcised 1 week ago - tremendous pain while retracting for cleaning - ? in spite of prescription pain meds :

Adult circumcision is more painful than in the children.
Ideally there should be least possible pain after 1 week after the circumcision- not tremendous at all.

Tremendous pain may be indicative of the following things:
-Tight scarification of healing
-mild infection
-increased sensitivity of the glans and the remaining foreskin as they are exposed to the atmosphere for the first time, if you had tight phimosis.
-Everyone's pain tolerance is different.
-Skin getting adherent to the glans.

Few things, I would like to know:
-What was the indication of circumcision ?
-Are you Diabetic ?
-Is there redness, oozing of liquid or pus ?
-If you are willing and if you can please post / attach the picture of the area for me to see exact position the wound is in.

Till then I would like to give a few suggestions:
Continue the Pain medicines round the clock.
You may apply antibiotic ointments like Povidone iodine or Soframycin or so, to keep the area sooth, soft and supple so that the pain during retraction is reduced, facilitates the movement.
Antibiotic if there is redness or oozing.

I hope this answer helps you. Please feel free to ask for further, I would be happy to assist you more.

Hi Dr, thank you for your prompt reply. I did have tight phimosis. The surgeon recommended the procedure due to the tightening of the foreskin , very difficult to retract fully and would sometimes be very hard to retract back. Yes I do have type II diabetes, however it is being controlled now.Prior to surgery I was one of those that wasn't fully blessed, now after the procedure it is almost non-extent . Retracting back up in like a turtle that doesn't want to come out, will this be permanent

Brief Answer:
Post a picture please if possible..

Detailed Answer:
Thanks for your feedback.

It is good to know that the Diabetes II is under control, but it does cause a delay in healing process to some extent.

I hope the full extra / redundant foreskin is excised. If it is so the glans should be practically fully visible and there is no necessity to retract the skin, as there wont be any, or if present should be minimal only.

Take care of Diabetes and get a treatment as suggested.
