Brief Answer:
need to look at the cause

Detailed Answer:
Hi
Thanks for your query.

There can be multiple causes if not producing enough blood in body after heart surgery. Like blood destruction by valve, low iron or vitamins, infections, etc.

The attachment you have provided is not opening. Can you please send it by email to YYYY@YYYY with ATTN: Dr Indranil Ghosh

I would recommend blood reticulocyte count with good peripheral smear, serum iron, TIBC, ferritin, B12 and folate at the least to get a basic idea of what is happening here.

I will be waiting for you to resend the attachment and any of the above results if available.

Regards

Brief Answer:
This is anemia due to destruction of Red cells by valve

Detailed Answer:
Oral iron and folate replacement should be provided, although blood transfusion may be required if this is not enough.

As there is a significant paravalvular leak, which causes the red cells to be under stress and get destroyed, reoperation may be required to correct it.
