Question: I have been married since last 4 years and am trying since last 10 months but no success therefore i did semen analysis..It shows that I have a sperm count of 5 million and they are 20% motile sluggishly linear progressive. I have been prescribed fertomid 25 mg and ubiq 100 for 60 days. Kindly let me know the following: 1) How much is our chance of naturally conceiving? 2) If chances are remote, should we opt for ivf or iui? what are the success chances in both? How much is the chance of birth defects in both ivf and iui?

I read your query and I understand your concerns.

Following is my reply

1. Unfortunately your sperm count is too low for an IUI.

2. You need to undergo ICSI ( Intracytoplasmic sperm injection ). Which is an extended version of IVF especially for men with low sperm count.

3. Please see an IVF centre for the same.

4. Success is around 50 percent in such cases. Success also varies depending upon your wife's egg quality when eggs are taken out.

5. Please repeat your semen analysis in a different standard laboratory after 3 months to check if there is any improvement.

6. Meanwhile continue with medicines prescribed.

1) In that case, we will start looking for nearby ivf centres. We would want to do ivf at a centre which is equipped with all latest equipments, technologies and facilities. Since we are new to icsi, kindly guide me on such latest equipments, technologies and facilities so that I can inquire about the same at ivf centres.

2) I am totally committed in increasing my sperm count and motility. Therefore, kindly recommend me a good daily routine that I can follow to increase my sperm count and motility. Also, please recommend necessary diet for the same.

3) I am anxious right now because of the results of my semen analysis. Therefore please guide me on what precautions should I take to make sure that my count does not decrease further.

Hi,

1) Let me know which place you are in. I will give address of a good IVF centre in that town.

2) Please have diet low in sugar and fat. Stop drinking alcohol and smoking if you do.

3) Continue medicines you are taking.

4) Wear loose pant and undergarments. Dont use very hot water for bath. Do regular excercise and dont sit in a place for along time.

Please also let me know the following: if I continue to live a disciplined life as per your mentioned instructions, how much time it will take for my sperm count and motility levels to get to normal range? This will help me know how much I need to wait for being able to naturally conceive?
