Question: I have lost consciousness two nights in a row whilst coughing,injuring myself on both occations.Very worried.

From the available straight forward explanation for your two episodes of loss of consciousness mostly due to is cough syncope.

It is common symptom in patients with COPD or asthma and in those who have severe cough of any reason.

It happens because there is excessive rise of pressure in your chest while coughing which disturbs the blood supply to brain.

So it can be concluded that it is cough related problem and likely to disappear with proper treatment of cough.

For this purpose you are requested to meet chest physician if the ENT has already cleared you. I understand that the problem was not there at time of your ENT visit but most of the ENT conditions can be visualized with simple observation.

Thank you for your clear response,.
May I ask, what is the connection between the very tight breathing ( sounds like croup) and the possible COPD that you explained. I cough a lot without losing consciousness but when I am suffering from the laboured breathing then only a short bout of coughing will render me senseless. Are there specific tests to establish cause and is it simply treatable.
I have had no health problems other than suffering from chronic Urticaria in recent years,I also experienced some laboured breathing at times along with the hives.
My only Drug history is one of using Cocaine and cannabis between 2006 and 2010.
Could that be connected? only a little cannabis but a fair amount of the afore mentioned.

Thanks for reverting back to me.

Both the conditions are very different. The tight breathing or breathlessness is just a symptom of many disorders including COPD.
COPD (chronic obstructive pulmonary disease) is a chronic respiratory disorder which includes both emphysema and chronic bronchitis. In this condition patient has problem with breathlessness, labored breathing, fatigue on exertion, limitation of activity among others.
You need not worry about these disorders as it seems that, you are not having this from the available history. As you did not mention smoking it is unlikely to occur as more than 95% of the patient with COPD has chronic smoking problem and the rest develop it due to some genetic disorders.

Presence of urticaria, labored breathing indicates toward allergic problem which may include asthma. A simple test of spirometry will reveal the true nature of the problem. It will also help you in differentiating from COPD.

Although cocaine can be related with the problem but again with spirometry we can identify. Regarding cannabis it does not affect any respiratory function.

To conclude again it is better step to meet a chest physician as soon as possible. A simple chest X ray along with spirometry likely to make diagnosis clear. In extreme cases there may be need of Computed tomography of chest.
