What Causes Liquid Discharge From Tubectomy Incision Site?

Question: Tubectomy has been done for my wife on 09-Feb-2014. Some liquid is coming out at the operated place even for today since one month. We visited the doctor who performed the operation and at every visit the doctor is suggesting some ointment for 5 days. But the problem did not resolved yet. Please can let us know what could be the problem and the solution for this.

Brief Answer: Do not worry. Detailed Answer: Hello and welcome to HCM, I am Dr Nilajkumar a consultant gynecologist and I will be helping you in your queries. I understand that a tubectomy surgery has been done and it is more than a one and half month ago. Some liquid is coming out. It seems that an infection has occurred or it is a reaction of the body to the suture material. Kindly provide information about which was the method of tubectomy open or laproscopy and also the color of the liquid and total amount in one day. Also it will help to know if she has other symptoms like fever, redness around the wound site, any gaping of wound. Also provide a list of antibiotics that she has taken and names of ointments. As of now, I suggest that you get a wound swab tested for culture and sensitivity to see which is the organism causing it. Most likely during the time of collection of wound swab, the doctor will try to express the liquid out by squeezing the area. Let all the liquid drain out. If there is a collection inside it delays healing. Get her blood sugar tested. If the liquid is coming out in excessive amounts like soaking the dressings completely then an ultrasound will be needed. She will need a daily dressing if the discharge is not clearing. For this either go to a doctor that has all sterile facilities or do at home. Use johnsons sterile dressings and wash your hands completely before doing it. Clean the part completely with betadine lotion and then clean it with a dry cotton. Use johnsons sterile dressings to do all this and keep it in a boiled and dried plate when you do it at home. If it is a small hole then apply mupirocin ointment and cover with gauze and sterile cotton and apply sticking plaster. Ask her to lie on her stomach as much as possible to allow the liquid to drain out. Once the liquid stops coming, there is no need to express it. Just clean with betadine lotion and dry gauze and apply sticking. Once there is formation of a scab, just clean with spirit and keep open. Another important issue is to prevent the wound from getting wet. That is do not allow any water to enter the wound as it will delay healing and also may aggravate infection. It is better to get all this done at a doctors place for a few days as sterile precautions are not possible at home. Once the swab reports are available, we may be able to decide the antibiotic she will need to kill the organism. Until then you may get a broad spectrum antibiotic prescribed by a doctor till reports are available. She will need this otherwise it may not heal. Also start vitamin C, tablet B complex and tablet serratiopeptidase to promote healing. Give her a complete diet of vegetables, fruits, cereals and sprouted beans, egg, milk and meat [ if non vegetarian]. This promotes healing.
