What Causes Left Sided Pain Under The Ribs?

Question: Hi
I'm a 41yo female; experiencing left sided pain under my ribs; radiating to left side of back near shoulder blade
Pain is sharp, stabbing, intermittent
Also have fatigue, nausea & stomach boating
Accompanied by dull ache in left lower back x 2wks

could be pancreatitis or renal stones

Detailed Answer:
Hi XXXX

There are three possibilities for your symptoms and they also have different investigations to confirm.

1- pancreatitis is the most likely cause, as you have a radiating pain to the back also. For confirmation, you need to get your serum amylase and lipase levels done. Treatment will be bland diet and enzyme supplements.
2- renal stone is the next possibility as your pain is sharp and intermittent with nausea. This will need an ultrasound of your abdomen for confirmation. Treatment will depend upon the no., size and location of the stone.
3- gastritis is another possibilities as you also have stomach bloating and nausea. It may even be present along with the above two. Diagnosis is either by a normal report in the above two tests or by endoscopy.

I would order for the aforementioned blood tests and ultrasound scan before proceeding further for endoscopy. These test will help us clinch the diagnosis. In the meanwhile, I would start you on PPI tablets like pantaprazole or omeprazole and antacids. Avoiding tea, coffee and soda drinks and spicy foods. Your local doctor will know about those tests.
